Mechanism

Points versus volume

Everything downstream — what each treats, what it feels like, what can go wrong — follows from this one difference in how the heat is distributed.

Focused ultrasound converges acoustic energy on a focal point below the surface. The point gets hot enough for collagen to contract and a healing response to start; the tissue between points, and the surface above them, is largely spared. Depth is set mechanically by the transducer or cartridge — 1.5 mm, 3.0 mm and 4.5 mm are the standard set — so the operator is choosing a layer rather than a temperature. The 4.5 mm depth reaches the superficial musculoaponeurotic system, the fibrous layer a surgical facelift repositions, which is why focused ultrasound is the mechanism associated with lifting an edge.

Radiofrequency passes current through tissue, which heats because it resists the current. The current spreads rather than converging, so a broad region of dermis warms together. The skin surface is protected by active cooling at the tip, which is what allows the layer beneath to reach a useful temperature without injuring the layer above. The result is diffuse collagen remodeling across a region, which changes overall tension and surface quality at the same time — but does not concentrate anything at a specific depth.

The one-sentence version. If you can point at the problem, focused ultrasound is the more direct answer. If you have to gesture at an area, radiofrequency is.

This is also why the two are complementary rather than competing, and why clinics sell them together in packages. What it does not justify is the common sales framing in which radiofrequency is the “entry” treatment and focused ultrasound the “premium” one. They are not rungs on a ladder, and someone whose complaint is crepey cheek texture will get a poorer result from the more expensive ultrasound treatment than from the cheaper radiofrequency one.

Full amounts including the list and standard tiers, and the smaller and larger doses, are on the price page. Cost per session differs by roughly ten to one across the five platforms; how often either needs repeating is decided at consultation.

The row worth pausing on is imaging. Only one of the five platforms shows the operator the tissue layer before delivering energy. That is a genuine functional difference and it is what the Ultherapy Prime premium buys — not better ultrasound, and not a different mechanism. Whether it is worth the difference depends on your anatomy and treatment history: it matters more if you have filler or threads at depth, and less on a straightforward face treated by an experienced operator.

The references on this page do not directly compare focused ultrasound with radiofrequency for facial laxity. They cannot establish that one is superior. This page explains differences in mechanism; choosing a treatment also requires assessment of your skin and the specific device. If a clinic or article claims superiority, ask for the study and check its devices, patient group, outcomes and limitations.

Including the two nobody selling a device wants to answer: which has better evidence, and what if neither is right for you.

What is the difference between HIFU and RF skin tightening?

HIFU concentrates ultrasound energy into small focal points at fixed depths, while radiofrequency heats a broad volume of dermis at once — that difference in heating pattern is the whole distinction. Focused ultrasound produces discrete columns of thermal change, including at 4.5 mm where the superficial musculoaponeurotic system sits, which is why it does more for a defined edge such as a jawline. Radiofrequency warms the dermis diffusely, so it does more for overall looseness and skin texture together. They are not tiers of the same treatment.

Which is better, Ultherapy or Thermage?

Neither Ultherapy Prime nor Thermage is better in general, because they answer different complaints: choose focused ultrasound for a lost jawline, jowls or sagging under the chin, and radiofrequency for diffuse looseness and crepey texture across a wider area. If you cannot decide which describes you, the useful test is what you do in front of a mirror. Pulling upward at the jaw and liking the result points to ultrasound; pressing the cheek flat and wanting the surface to look better points to radiofrequency. A physician examining you settles it properly.

Should I do HIFU or RF first?

Do whichever addresses your main complaint first, and if both apply, do the focused ultrasound first. The reasoning is practical: the ultrasound result is judged at two to three months, and having it first gives a clean read on whether tissue tension was actually the problem before a second variable is added. Doing both in one visit is also legitimate and is what the clinic’s combined packages do, at the cost of not knowing which one produced the change.

Is HIFU cheaper than radiofrequency in Korea?

HIFU is dramatically cheaper than radiofrequency in Korea at the low end, and roughly comparable at the high end, so the category answer is no. Can HIFU and radiofrequency be combined in one session?

HIFU and radiofrequency are routinely combined in one session, and the clinic’s VIP packages are built on exactly that combination because the two mechanisms address different tissue behavior. The trade-off is that stacking two heat-based treatments increases swelling and, in a thin face, the risk of fat loss, so it is a physician’s judgment rather than a default. It also removes your ability to tell which treatment produced the result. That matters if you intend to repeat only one of them.

Does HIFU or RF work better for a double chin?

Focused ultrasound is generally the better-matched treatment under the chin, because the complaint there is usually tissue laxity at depth rather than surface skin quality, and the 4.5 mm depth reaches the fibrous layer involved. If the submental fullness is fat and not lax skin, neither treatment is the right answer — that is a different problem, and fat-reduction approaches address it rather than a lifting treatment. Have someone determine which it is before booking either.

Which treatment has better evidence behind it?

Both mechanisms rest on modest published literature, and neither is supported by the volume of large randomized trials the marketing implies. Two references sit at the foot of this page: a review of microfocused ultrasound, and a clinical-applications review of monopolar radiofrequency. Both are narrative reviews and case series rather than controlled comparisons, which is the kind of source available. The references cited here do not provide a direct comparison establishing which treatment is better for your face. If a page claims one is superior, ask for the study and check which devices, patients and outcomes it compared.

Is HIFU or RF safer?

HIFU and radiofrequency have different risks and contraindications; this page cannot rank one as safer for every patient. Tell the physician about any pacemaker, defibrillator or other implant before considering either treatment. Thermage and Ultherapy manufacturer instructions list active electronic implants as contraindications; do not assume focused ultrasound is an alternative simply because the implant is away from the face. Both can, rarely, cause loss of subcutaneous fat producing a visible hollow, and focused ultrasound carries a small risk of transient nerve effects. Neither is risk-free, and you should expect both the benefits and the risks to be set out before you agree to anything.

What if neither HIFU nor RF is right for me?

If your complaint is volume loss rather than laxity, neither treatment is the answer, and the options are volumizing approaches or accepting the change. If the laxity is advanced, a surgical opinion is the referral, rather than a course of energy treatments that will disappoint. If what bothers you is skin texture and pore appearance rather than tension, a skin booster such as Rejuran addresses that directly.
